Community Roots specializes in residential services for adults with intellectual and developmental disabilities under the HCBS-DD Medicaid Waiver in the Boulder and Broomfield County region. We work with individuals and their support team to determine the best residential setting which may include Family Caregiver, Host Home, or Independent Home. We spend ample time future planning with individuals and families to understand their specific needs and to develop creative placement options that are customized to support connection and fulfillment in their life.
